Get a printed directory in My Health Link

Want to print out your own list of doctors in your plan and area? Or receive a printed Evidence of Coverage or covered prescription drug list (formulary) in the mail? If you’re a Florida Blue Medicare Advantage member, you can get started right in My Health Link™ at floridablue.com/medicare.

Here’s how to create your personalized list of doctors:

• Log in to My Health Link at floridablue.com/medicare.
• Select Find Care, and choose Create Custom Directory from the dropdown menu. 
• 
• Select your plan from the dropdown menu.
•  
• 
• Choose your region and county.
• 
• Choose the provider types you want to include.
• 
• Download your directory!
• 

Here’s how to get your Evidence of Coverage (EOC) or prescription drug list (formulary) mailed to you:

• Log in to My Health Link at floridablue.com/medicare.
• Select My Plan, then choose Request EOC/Formulary. 
• 
• Then, choose which document you want to receive and for what year.
• Then click Request Document.

Never used My Health Link before? It’s easy! Here’s how to get started:

• Have your Florida Blue Medicare member ID card available
• Visit floridablue.com/medicare
• Click Log in
• Select New Member Registration and enter your information (including your member number from your Member ID card) to create an account
