From the comfort of home

As a Florida Blue Medicare Advantage member, you can get a personalized health assessment with a nurse practitioner in the comfort of your own home. This visit is no extra cost to you, and you can even earn HealthyBlue Rewards just for completing this visit.

  • One-time annual visit

    A nurse practitioner will come to your home and complete a focused physical exam. During this exam, they will review your medications with you and answer any health questions you may have. They’ll also assess any risks you may face in your home. They will share the results with you and your primary care doctor so you can discuss them at your next visit. 

    Remember: This doesn’t replace the regular care you get from your primary care doctor. It’s still important to get your Annual Wellness Visit with your doctor!
  • If you’re eligible for an in-home assessment, we’ll send you a letter or call you to set up your visit.

    Florida Blue Medicare works with several providers to offer you home health assessments. Depending on which provider you see, the nurse practitioner may come to your home for the visit or see you during a telehealth appointment.
