If you have a cardiovascular disorder, chronic heart failure, or diabetes, a C-SNP plan can do more than cover basics. Florida Blue Medicare offers a dedicated care team working together for you, doctors trained in your specific condition, and extra coverage tailored to your actual needs.
Benefits of the Florida Blue Medicare C-SNP Plan

- Get up to $105 a month for approved healthy food*, transportation, and over the-counter (OTC) items
- $0 Copay on primary care visits and $15 copay for specialists

- $0 Vision exam copay and $225 vision allowance per year for lenses, frames, or contacts

C-SNP plans offer proactive care coordination and access to specialized providers to help manage conditions.
Your dedicated care team is here to support you every step of the way. You'll work one-on-one with a registered nurse case manager who knows your health story and coordinates all your care. Together, you'll create a personalized care plan based on your goals and preferences—and your team regularly checks in to make sure it's working for you. Depending on your needs, your team may also include specialists, social workers, diabetes educators, pharmacy support, and behavioral health experts. They handle the details: scheduling appointments, coordinating referrals, arranging transportation or community services, and answering your questions anytime. Plus, they proactively reach out to support your medication adherence and overall health.
*If you’re diagnosed with any of the chronic condition(s) listed below and meet certain criteria, you may be eligible for special supplemental benefits for the chronically ill.
- Cardiovascular Disorders, Chronic Heart Failure, Diabetes Mellitus.
- Eligibility will be identified by receipt of a provider-submitted institutional or professional claim that includes diagnostic codes indicating one or more of the above listed chronic conditions.
- In addition, members must have completed a Health Risk Assessment (HRA) for the current plan year to be eligible.
