HealthyBlue Rewards & Loyalty Program (HealthyBlue) restrictions and limitations may apply. Members must be registered in HealthyBlue to be considered eligible for the HealthyBlue program. Eligible members can earn and redeem rewards for completing certain preventive screenings and tests between January 1 and December 31, 2019. Only current Florida Blue Medicare Advantage members in one of these plans is considered eligible for the 2019 HealthyBlue Reward & Loyalty (HealthyBlue) program: BlueMedicare Classic (HMO), BlueMedicare Classic Plus (HMO), BlueMedicare Premier (HMO) BlueMedicare Complete (HMO SNP), BlueMedicare Choice (Regional PPO), BlueMedicare Select (Local PPO), BlueMedicare Value (Local PPO) and BlueMedicare Group Retiree (EGWP PPO). Participation in HealthyBlue is voluntary and offered at no cost to you. The HealthyBlue program is not available to BlueMedicare Preferred (HMO), BlueMedicare Preferred HMO (HMO SNP), BlueMedicare Preferred POS (HMO POS) and Florida Health Care Plan (FHCP) members at this time.

Important Dates to Know

  • You can only earn HealthyBlue points by completing eligible health activities between January 1 and December 31, 2019.
  • The last day to redeem any HealthyBlue points earned in 2019 is December 31, 2019 by 12 PM EST. If you complete an eligible health activity on December 31, call us to report it so you’ll get credit and be able to earn your reward.

Program Rules

  • You can redeem your HealthyBlue points for gift cards from a variety of popular merchants. HealthyBlue points cannot be redeemed for cash, and may not be used to pay for your health care premium or cost sharing (deductible, coinsurance or copayment).
  • Eligible plan members can only earn one (1) reward per one (1) eligible health activity per calendar year.
  • Eligible plan members can only be rewarded Gold or Silver status in calendar year 2019. A member cannot have both Gold and Silver status in calendar year 2019. Only one of three Health Programs (Men’s, Women’s or Diabetes can be redeemed for an award and used toward Gold or Silver status.
  • Eligible plan members must achieve Bronze status before achieving Gold or Silver status. Bronze status is achieved by completing the online Welcome questions when logging into the member’s online account for the first time in 2019 or completing the member Welcome questions by phone. Members can call <1-855-209-2427> 8 AM to 6 PM, Monday through Friday all year except national and company holidays.
  • You receive rewards based on the claims we receive for your health activity, or when you give us the details for a completed health activity.
  • Eligible plan members are responsible for accurately and honestly representing health activity they are reporting.

Current health activities eligible for HealthyBlue Rewards & Loyalty include:

  • Annual Wellness Visit or In-Home Assessment (awards will only be given for one not both activities).
    • Your Annual Wellness Visit is not the same as an annual physical or visit when you are sick. It’s a special Medicare-covered visit. You can only receive a reward for getting an Annual Wellness Visit, not a physical or sick visit.
    • Similar to an Annual Wellness Visit, an In-Home Assessment is conducted by a certified nurse practitioner, from one of Florida Blue’s approved vendors, who will visit you in your home, or if needed another convenient location, to gain an understanding of your current overall health and health goals, and to provide you with preventive steps. Florida Blue offers health assessments, at no additional cost to you, to help members understand what they can do to stay healthy, and reduce your chance of illness.
      • Breast Cancer Screening (preventive mammogram)
      • Bone Density Screening or Treatment for Osteoporosis
      • The osteoporosis management reward is for women who have received a Bone Density screening or are taking qualified medications for osteoporosis.
      • Colon Cancer Screening
        • There are several types of colon cancer screenings that qualify for rewards. However, you may only earn one reward for completing an eligible colon cancer screening per calendar year.
      • Annual Flu Shot*
      • Diabetic Retinal Eye Exam
      • Diabetic Blood Test (A1C)
      • Diabetic Urine Test for Protein
      • Women’s Health Program: By completing these screenings and health program content below, you can achieve a higher member level and can receive an additional reward points.
